- Limited to 5 visits, per person, per policy year.
- Treatment must follow Emergency Room or Urgent Care Center treatment and must begin within 30 days of initial onset of accidental injury in most states.
Outpatient Surgery Benefit

- Limited to 1 surgery, per person, per policy year.
